    • Service de Chirurgie Orthopédique et Chirurgie du Sport, Hôpital Pasteur, Nice, France with Professor Pascal Boileau, held in July 2018
      Professor Pascal Boileau’s name is unavoidable when it comes to shoulder surgery worldwide. With so many published works, lectures, courses, prostheses and techniques developed by him, it became obvious the need to learn closely from this “great master” who is leaving his mark on the new generation of Shoulder Surgeons.

      Professor Boileau graduated in Medicine in 1989. He completed his specialization in Orthopedics and Traumatology in 1993. Throughout his medical career, he has received numerous awards for his roles and scientific work. He is currently the Director of the Orthopedic Surgery and Sports Surgery Department at the Pasteur 2 Hospital in Nice (since 1999), Director of the University Institute of Locomotion and Sport (since 2008) and Head of Outpatient Surgery at the Nice University Hospital Centre. He specializes in treating athletes with shoulder instability and degenerative pathologies.

      The objective of this internship was to improve the clinical evaluation of patients with shoulder pathology, understand other forms of diagnosis and therapeutic decision-making, learn new surgical approaches to the shoulder, assimilate the steps of Arthroscopic Laterjet and understand the concept of BIO-RSA, both techniques developed by Professor Boileau.

    • Roth McFarlane Hand and Upper Limb Centre, London, Canada, with Professor George Athwal, held in April 2019
      Roth-McFarlane Hand and Upper Limb Centre is a world-renowned and respected centre with a level of excellence in education, research, diagnosis and treatment. They are the largest upper limb centre in Canada, with patients coming from all over the world, treating over 40,000 patients per year. Those who follow and attend courses and conferences on shoulder pathology will realise that Professor George Athwal represents a new generation of shoulder surgeons who make research and scientific knowledge their trademark. He has been known for his captivating lectures but, above all, for his knowledge of prosthetics and prosthetic revisions, and is currently one of the world’s leading references in this area.

      The Hon. Professor George Athwal graduated in Medicine from the University of British Columbia. After completing his internship in Orthopaedics at Queen’s University, he spent 2 years subspecializing in upper limb surgery, 1 year at the Hospital for Special Surgery in New York and 1 year at the Mayo Clinic. In 2005, he joined the team at the Roth-McFarlane Hand and Upper Limb Centre. He is actively involved in clinical and biomechanical research projects, both in the shoulder and elbow areas. He has published extensively (more than 200 articles), is a speaker at many conferences and courses and has received several awards throughout his not-so-long career.

      The aim of the internship was to improve and diversify theoretical and practical knowledge of prosthetic shoulder surgery, both in primary and secondary arthroplasties. A second objective was to be able to contact the biomechanics laboratory of this centre.

    • Fortius Clinic, London, England, with Drs. Andy Williams and James Calder, held in October 2019
      Internship in Orthopedics and Sports Traumatology, allowing the student to learn from renowned professionals linked to sports, who treat football and rugby players from the best leagues in the world, being the greatest references in this area.

      Professor James Calder is an internationally recognized foot and ankle surgeon in the treatment of sports injuries. He works with athletes and sports professionals from the main English, Spanish and French football clubs, as well as the main English rugby clubs and also British athletes linked to gymnastics, ballet and hockey, among others. He maintains a research project in the Bioengineering department of Imperial College, London. He is currently the president of the European Society of Sports Traumatology, Knee Surgery & Arthroscopy – Ankle & Foot Associates (ESSKA-AFAS). Professor James Calder is a foot and ankle surgeon internationally recognized in the treatment of sports injuries

      . Dr. Andy Williams is one of the most renowned knee surgeons in the world, both for the many famous football players he has operated on and for his lectures and published scientific articles. He is a world reference in the reconstruction of the anterior cruciate ligament, posterior cruciate ligament and multi-ligament injuries.

      The objectives of this internship were to enrich theoretical and practical knowledge in the area of ​​sports traumatology, improve the clinical assessment of these patients and the ability to interpret imaging studies, as well as learn different arthroscopic and open techniques for their treatment.

São o maior centro de membro superior do Canadá, sendo que os doentes são provenientes de todo o mundo, tratando mais de 40 000 doentes por ano. Quem acompanha e frequenta cursos e congressos sobre patologia do ombro, percebe que o Professor George Athwal representa uma nova geração de cirurgiões de ombro que fazem da investigação e conhecimento científico a sua imagem de marca.     O objetivo do estágio consistiu no aperfeiçoamento e diversificação do conhecimento teórico e prático da cirurgia protésica do ombro, tanto nas artroplastias primárias como secundárias. Um segundo objetivo foi poder contactar com o laboratório de biomecânica deste centro.

Trabalha com atletas e profissionais desportivos dos principais clubes ingleses, espanhóis e franceses de futebol, assim como dos principais clubes ingleses de rubby e ainda atletas britânicos ligados à ginástica, ballet e hóquei, entre outros. Mantém um projeto de investigação no departamento de Bioengenharia do Imperial College, em Londres. Sr. Dr. Andy Williams é um dos cirurgiões de joelho mais conhecidos em todo o mundo, tanto pelos inúmeros jogadores de futebol conhecidos que já operou como pelas suas palestras e artigos científicos publicados. É uma referência mundial na reconstrução do ligamento cruzado anterior, cruzado posterior e em lesões multiligamentares.

    Os objetivos deste estágio consistiram em enriquecer os conhecimentos teóricos e práticos na área de traumatologia desportiva, melhorar a avaliação clínica destes doentes e a capacidade de interpretação dos estudos imagiológicos, bem como, aprender diferentes técnicas artroscópicas e abertas para o seu tratamento.

    • Best poster award at the National Congress of the Portuguese Society of Orthopedics and Traumatology (SPOT), in October 2017 with the work “Crouzon Syndrome. A rare cervical spine kyphotic abnormality”

    • Award for the Best Clinical Case in which he was a co-author at the National Foot and Ankle Congress, in May 2016 with the work “Gigantism of the Foot”
    • Best Free Communication Award at the 40th National Congress of the Portuguese Society of Orthopedics and Traumatology (SPOT) for having been co-author of the work “Glenoid notching after reversed shoulder arthroplasty: influence of the cervico-diaphyseal angle of the humeral component”, in October 2021.

    • uprascapular Neuropathy In A Young Male Handball Player: Case Report, published in the International Journal of Shoulder Surgery
      Abstract: Background: Suprascapular neuropathy is an uncommon but a very disabling condition. If not diagnosed early, it can lead to irreversible changes. Their prevalence is higher among illnesses and active population, mainly in overhead athletes. Methods: A 22-year-old male, who is a handball player, reported right shoulder pain, fatigue and discomfort after one month holding a backpack during hiking on his vacations. He did not remember any trauma to the right shoulder. Clinically, he had normal active and passive range of motion but moderate weakness of the right shoulder external rotator muscles. At inspection he had marked atrophy of supraspinatus and infraspinatus muscles. MRI demonstrated a superior labrum from anterior to posterior lesion with large multiloculated paralabral cyst and electromyography was consistent with compression of suprascapular nerve. Results: The patient was submitted to arthroscopic labral repair and decompression of suprascapular nerve. Six months after surgery he has no pain and limitation and is recovering strength. Conclusions: Paralabral cysts are described in the literature as causing compression on spinoglenoid notch and thus coursing with infraspinatus atrophy. This case demonstrates an unusual presentation with both supra and infraspinatus muscles atrophy. Early recognition of these injuries is crucial because complication and morbidity rates are high with delayed diagnosis, especially in athlete population.
    • Tuberosity Consolidation in Reverse Shoulder Arthroplasty After Proximal Humerus Fracture: Is There an Improvement in Functional Outcomes?, published in the Brazilian Journal of Orthopedics
      Abstract: Objective: To compare the functional outcomes between patients with complex proximal humerus fractures undergoing reverse arthroplasty with consolidated tuberosities and non-consolidated tuberosities. The secondary objective was to determine the rate of tuberosity consolidation with this type of prosthesis. Methods: Cohort study, retrospective, with prospective data collection. Twenty-eight patients met the inclusion criteria: age over 65 years, undergoing reverse shoulder prosthesis for complex proximal humerus fracture (3 or 4 parts, according to Neer), minimum follow-up time of 24 months. At six months, all patients were radiographically evaluated for tuberosity consolidation and divided into 2 groups: group with consolidated tuberosities and group with non-consolidated tuberosities. Functional assessment was performed according to the Constant scoring system, active mobility and Visual Analogue Scale (VAS) at the date of the last consultation. All complications were recorded. Results: Tuberosity consolidation occurred in 21 patients (76.3%). Statistically significant differences were observed in the Constant scoring system (p<0.001), anterior elevation (p=0.020), internal rotation (p=0.001) and external rotation (p=0.003) when comparing the group with consolidated tuberosities with the group with non-consolidated tuberosities. There were no significant differences in VAS between the 2 groups. Conclusion: Tuberosity consolidation results in improved functional results in patients undergoing reverse shoulder arthroplasty as treatment for complex proximal humerus fractures in the elderly.
    • Early Debridement, Antibiotics And Implant Retention (Dair) In Patients With Suspected Acute Infection After Hip Or Knee Arthroplasty – Safe, Effective And Without Negative Functional Impact, published in the Journal of Bone and Joint Infection
      Abstract: Introduction: Debridement, antibiotics and implant retention (DAIR) is known to be effective in treating acute periprosthetic joint infection (PJI). However, deciding to perform additional surgery in the early postoperative period may be challenging as there is the concern of adding morbidity and clinical presentation is often subtle. We mean to assess the impact of early DAIR on final functional outcome. Methods: A case-control comparison was performed between patients who underwent DAIR for suspected PJI between 2010-2016 and controls randomly selected (1:2 ratio) from a list of primary joint replacements. Patients were matched for 93anatomic site, age, gender, American Society of Anesthesiologists (ASA) classification, body mass index and follow-up time. The outcome of surgical treatment and complications were assessed and Hip disability and Osteoarthritis Outcome Score (HOOS) or Knee injury and Osteoarthritis Outcome Score (KOOS) were performed. Results: Thirty-eight cases were included at a mean follow-up of 42 months. Infection was not confirmed in one patient. There was one infection-related death and three other cases of treatment failure that required a two-stage revision. Overall success rate was 89.2%. There were no significant patient reported differences regarding final functional outcome between both groups: pain 91±6 vs. 87±13; other symptoms 90±8 vs. 90±9; activities of day living 86±8 vs. 85±14; sport 63±13 vs. 57±16; quality of life 78±17 vs. 76±16. Discussion: These findings support that DAIR for suspected acute PJI is safe, effective and causes no impact on final functional results. Thus, a low threshold for assuming infection and subsequent DAIR may be safely adopted in the early postoperative period.
    • First Carpometacarpal Joint Osteoarthritis: Trapeziectomy With Ligament Reconstruction And Tendon Interposition Versus Ball-And-Socket Arthroplasty, published in Acta Orthopaedica Belgica
      Abstract: Background: The objective of this study was to compare trapeziectomy with ligament reconstruction and tendon interposition versus ball-and-socket arthroplasty for first carpometacarpal joint osteoarthritis treatment. Methods: In this retrospective study, a total of 98 patients were included between January 2011 and December 2015: 27 patients were submitted to ligament reconstruction and tendon interposition technique (group A) (mean age of 62.01 years-old) and 71 patients to prosthesis replacement (group B) (mean age of 62.14 years-old). Clinical assessment was employed at the last appointment before discharge. Results: Groups did not significantly differ regarding VAS (group A: 1.04; group B: 1.03; p=0.486) and QuickDASH (group A: 13.72; group B: 11.76; p=0.290). First carpometacarpal joint osteoarthritis arthroplasty was associated with greater strength when performing key-pinch (group A: 4.44KgF; group B: 5.20KgF; p=0.031) and grip (group A: 19.25KgF; group B: 22.35; p<0.001). There were no significant differences between groups concerning complications (p=0.502, Fisher’s exact test). Conclusion: There were no differences in terms of clinical outcome and complication rate between the two groups for patients with first carpometacarpal joint osteoarthritis Eaton stage II and III. However, patients with prosthesis have greater key-pinch and grip-strength.
    • Metatarsophalangeal Arthroplasty for the Treatment of Hallux Rigidus, published in the Portuguese Journal of Orthopedics and Traumatology
      Abstract: Objective: To prospectively evaluate, clinically and radiologically, patients undergoing MTP-1 arthroplasty with the same prosthesis for the treatment of hallux rigidus . Material and Methods: Prospective study conducted between 2009 and 2015, in which we included all MTP-1 arthroplasties performed for the treatment of patients with hallux rigidus in stages III and IV. Patients were clinically evaluated using the visual analogue pain scale and the American Orthopaedic Foot and Ankle Society Hallux Metatarsophalangeal-Interphalangeal (AOFAS-HMI) scale, both performed prior to surgery and at the last consultation before discharge. Patients were also asked whether they returned to their previous active life and whether they would undergo the same intervention again. A radiological evaluation was also performed to look for signs of detachment, the survival rate was calculated and all complications were recorded. Results: During this period, 26 arthroplasties were performed in 25 patients, with a mean follow-up time of 80.7 (26-110) months. All patients reported a significant improvement in pain and functional results after surgery compared to the preoperative period, both on the AOFAS-HMI scale (89.7% vs. 43.2 points; p<0.001) and on the VAS (1.6 vs. 8.0 points; p<0.001). The survival rate was 100%, all patients returned to their previous active life and responded that they underwent the same surgery again. There was only one complication: an early infection. Conclusion: MTF-1 arthroplasty is a safe and effective technique in the treatment of hallux rigidus in stages III and IV with good functional results and low complication rates.

      Abstract: Abstract: Isolated lesser tuberosity fractures are rare, misdiagnosed, and their treatment remains controversial. This case reports the diagnosis, therapeutic options and clinical and functional outcomes of a 30-year-old male with a lesser tuberosity fracture, submitted to osteosynthesis with screws. The case presents an excellent clinical, functional and radiological outcome.
    • Arthrodesis Versus Arthroplasty Of The First Metatarsophalangeal Joint In The Treatment Of Hallux Rigidus: Comparative Study Of Appropriately Selected Patients, published in Revista Brasileira de Ortopedia
      Abstract: Abstract: Historical results of arthroplasty of the first metatarsophalangeal joint (1MTP) are relatively poor; However, improvements in the understanding of the normal foot biomechanics, implant materials and design currently make arthroplasty a reasonable option in appropriately selected patients. This study aimed to compare the clinical and radiographic results of 1MTP arthrodesis and arthroplasty in the treatment of hallux rigidus and to present a rationale for patient selection for arthroplasty. Methods: 36 patients (38 feet) with hallux rigidus submitted to surgery (12 arthrodesis and 26 arthroplasties) were prospectively included in the study. Pain was assessed using visual analogue scale (VAS) and functional status using the AOFAS-HMI (American Orthopedic Foot and Ankle Society Hallux Metatarsophalangeal-Interphalangeal) scale. Complications and radiographic results were also analyzed and survival rates were calculated for both procedures Results: All patients reported significant improvement in pain and functional status after surgery. Patients submitted to arthroplasty had better functional results on the AOFAS-HMI scale (89.7 vs. 65.7 points; p<0.001) and better pain relief (VAS 1.6 vs. 3.9 points; p=0.002) when compared to the group submitted to arthrodesis. There was one case of infection in the arthroplasty group and 2 cases of pseudoarthrosis in the arthrodesis group. Conclusion: Arthrodesis provides pain relief and satisfactory results but alters the biomechanics of gait. Like arthrodesis, arthroplasty improves pain significantly, being a more physiological alternative to preserve foot biomechanics. While the two surgical methods yielded good clinical results, selected patients submitted to arthroplasty had better clinical scores and lower revision rates.
    • Horner Syndrome After Anterior Revision Surgery For Cervical Spondylotic Myelopathy: A Very Rare Complication – A Case Report, published in The Journal of Bone and Joint Surgery
      Summary: Case: We report a case of postoperative left-sided Horner syndrome (blepharoptosis, miosis, and anhidrosis) after revision surgery for cervical spondylotic myelopathy. A C4 corpectomy and an anterior cervical fusion from C3 to C5 were performed through a left longitudinal approach in a patient with a surgical history of an anterior discectomy and a fusion from C5 to C6. One year after the revision surgery, the patient had recovered from the anhidrosis and the miosis, but the blepharoptosis was not fully resolved. Conclusion: Horner syndrome resulting from surgical injury to the ipsilateral cervical sympathetic chain is a very rare complication of anterior spine surgery that has been reported when the lower cervical levels have been addressed. Awareness of this important cervical structure may help to avoid this complication
    • How To Repair An Immediately Post-Operative Anterior Cruciate Ligament Reconstruction Failure? About A Clinical Case, published in the International Journal of Research in Orthopedics
      Abstract: We present a 23-year-old male with an early anterior cruciate ligament (ACL) reconstruction failure due to lateral wall blow-out, diagnosed on day one post-op. We were able to perform the revision using the original graft, and maintaining the initial tibial fixation, revising only the femoral fixation. At 1-year follow-up the patient is asymptomatic. In the early ACL reconstruction failures due to femoral wall blowout, it is possible to perform the revision surgery using the same graft and maintaining the same tibial fixation. This avoids morbidity of a new graft harvest and the need for a new tibial tunnel
    • Chapter in International Book, “Work-Up And Management Of Infection In Shoulder Arthroplasty”, published in the book “Massive and Irreparable Rotator Cuff Tears From Basic Science to Advanced Treatments”
      Summary: Prosthetic Joint Infection (PJI) is an important complication after shoulder arthroplasty and is often associated with significant morbidity. However, clear signs and symptoms of infection are not always present. As such, it is mandatory to consider each painful shoulder arthroplasty as potentially infected until proven otherwise and to perform a systematic diagnostic investigation. However, the exact diagnostic and therapeutic strategies are still a matter of open debate. The authors present a critical review of the management of infection in shoulder arthroplasty considering their own experience.
    • Shoulder Arthroplasty Options for Glenohumeral Osteoarthritis in Young and Active Patients (< 60 Years Old): a Systematic Review, published in “The Journal of Shoulder and Elbow Arthroplasty”
      Abstract: Aim: This study aims to describe the shoulder arthroplasty options for young and active patients (<60 years old) with glenohumeral osteoarthritis. Methods: A systematic review of the literature was conducted by searching on Pubmed database. Studies that reported outcomes of patients with glenohumeral arthritis, younger than 60 years, who underwent shoulder arthroplasty [(Hemiarthroplasty (HA), Hemiarthroplasty with biological resurfacing (HABR), Total shoulder arthroplasty (TSA), Reversed total shoulder arthroplasty (RSA)] were included. Data include patient characteristics, surgical technique, range of motion, pain relief, outcome scores, functional improvement, complications, need for and time to revision. Results: A total of 1591 shoulders met the inclusion criteria. Shoulder arthroplasty provided improvements in terms of ROM on the 3 planes, forward flexion (FF), abduction (Abd) and external rotation (ER), in different proportions for each type of implant. ER postoperative values (vsRSA p = 0.049). Pain scores improved in all groups but no difference between them (p = 0.642). TSA and RSA groups had the best CS Δ (p = 0.012). HA group had higher complication rates (21.7%), RSA (19.4%, p = 0.034) and TSA (19.4%, p = 0.629) groups the lowest, and HABR had the highest rate of revisions (34.5%). Conclusions: HA had the highest rate of complications and HABR unacceptable rates of revision. These implants have been replaced by modern TSAs, with RSA reserved for complex cases. Surgeons should be aware of the common pitfalls of each option.

       

    • Surgical treatment for acute and displaced proximal humerus fractures in elderly patients: hemiarthroplasty vs. reverse shoulder arthroplasty, published in the Journal of Shoulder and Elbow Surgery
      Abstract: Background: Proximal humerus fractures (PHFs) are the third most common fractures in elderly patients. The best treatment option on this population is still a topic of discussion. Reverse shoulder arthroplasty (RSA) has increased popularity as a viable treatment option for these fractures. Yet, few studies demonstrate the risk factors for mortality after this procedure. Methods: The authors present a retrospective study including patients older than 75 y with acute and displaced PHFs primarily treated with RSA or hemiarthroplasty in a public hospital between January 2004 and January 2020. The exclusion criteria were pathologic fractures and more than 6 weeks to surgery. Survival curves were obtained using the Kaplan-Meier method and the log-rank test was performed to compare survival rates. Results: A total of 73 patients met the inclusion criteria. The mean age at the time of fracture and surgery was 78 y old, 10 males and 63 females, with a median clinical follow-up of 64 months (standard deviation 34). Forty-one patients (56%) had an American Society of Anesthesiologists score of 2. Twenty-eight patients were submitted to hemiarthroplasty and 45 to RSA. Regarding hemiarthroplasty, only one patient with hemiarthroplasty died within a year, and the 5 y survival rate was 70%. Concerning the RSA group, five patients died within a year, and the survival rate at 5 y was 66.2 %. The American Society of Anesthesiologists score ( P < .001) was the only risk factor identified for mortality at 5 y. Hemiarthroplasties had more prosthetic loosening compared with RSA ( P = .024). Three hemiarthroplasties were converted to RSA, and we verified 1 RSA infection. In the group of hemiarthroplasties, 56% returned to their normal daily living activities, while in the RSA 92% did that, representing a significant difference ( P = .007). Conclusion: RSA as a primary treatment for displaced PHFs had a high survival rate (88.9% at 1 year and 66.2% at 5 y) and better functional results comparing to patients treated with hemiarthroplasty. With proper patient selection, RSA is a safe procedure for the treatment of PHF, especially in an elderly population.

    • TOTAL VERTEBRECTOMY IN THE TREATMENT OF SPINAL TUMORS

      Abstract: Five patients with spinal tumors were presented: 3 with malignant tumors (chordoma, angiosarcoma and high-grade epithelioid sarcoma), 1 with an aggressive benign tumor (giant cell tumor) and 1 with a single metastatic vertebral lesion. All patients underwent single-level vertebrectomy: in 3 cases a posterior technique alone was used and in the other 2 cases a combined anterior and posterior approach. Complete tumor resection was achieved in 4 cases and at the time of last follow-up these patients were disease-free. Radiographic fusion was achieved in all cases. It was concluded that, despite being a very demanding technique in technical terms, total vertebrectomies allow complete recession of tumors involving the spine.
     
    • GIANT CELL TUMOR OF BONE: THE INFLUENCE OF DENOSUMAB IN SURGICAL TREATMENT

      Abstract: Retrospective study of 3 patients with GCT treated with denosumab: 2 cases in which surgery would involve high functional morbidity, even without complete tumor excision, and 1 case that was an unresectable recurrence. The 3 cases described underwent treatment with denosumab, with clear clinical and imaging improvement. The sclerosis resulting from the treatment appears to be beneficial, with a consequent reduction in surgical morbidity. It was concluded that denosumab has an important role in the treatment of unresectable or recurrent GCTs, since in addition to reducing symptoms in non-surgical cases, it can be used to reduce surgical morbidity in resectable cases. It remains to be clarified in which cases there is an unequivocal benefit of treatment with denosumab before surgery and the duration of treatment.

      Abstract: Study that included 34 patients undergoing osteosynthesis of isolated patellar fracture.       Abstract: Study of 29 patients with suspected acute infection of hip or knee arthroplasty undergoing surgical debridement with prosthesis preservation and replacement of mobile parts, who completed targeted antibiotic therapy. The results of this study demonstrated that early surgical debridement is an effective alternative without significant implications for the long-term functional outcome of arthroplasty. The authors argue that, given the complicated evolution of the wound in the early postoperative period, a low threshold for assuming a diagnosis of possible infection is beneficial. When in doubt, appropriate treatment should include aggressive surgical debridement, collection of samples for microbiological study and appropriate antibiotic therapy.

      Abstract: Study of 28 patients with complex fracture of the proximal humerus, undergoing inverted total shoulder arthroplasty. Radiological consolidation of the tuberosities was observed in 76% of patients. There was a statistically significant difference in the Constant score between the group with consolidation and the group without consolidation of the tuberosities. There was a statistically significant improvement in external rotation and anterior elevation of the shoulder.

      Abstract: Study of the epidemiology of spinal fractures observed in the emergency department of the Porto Hospital Center. Demographic data, fracture level, injury mechanism, type of treatment (conservative/surgical) and follow-up time were analyzed. This study highlights the existence of two major groups of spinal trauma: older female patients with type A1 osteoporotic fractures associated with minor trauma, without the need for surgery, and younger male patients, workers, victims of accidents, of greater violence, with fractures involving recession of the posterior wall and often associated with neurological deficits, requiring surgical intervention.

    Abstract: A study was presented whose objective was to understand the potential benefits of referral to a rehabilitation unit in patients suffering from proximal femur fracture. A total of 322 patients were included and divided into 2 groups: the group of patients referred to the Continuing Care Unit and those not referred. Despite the theoretical advantage of referral, this work demonstrated that survival, whether early or late, does not improve with entry into the network.

      Abstract: Study that included 154 patients undergoing primary hip arthroplasty. The aim of the study was to evaluate the variation of hemoglobin and consequent need for transfusion after surgery and to try to find variables that contribute to its reduction. It was concluded that preoperative hemoglobin assessment and compliance with transfusion protocols are important variables, which are potentially controllable and prevent the need for transfusion.

      Abstract: The authors present a literature review based on 5 cases of osteoid osteomas of the spine treated by radiofrequency or surgical ablation. It was concluded that although conventional treatment with resection of the lesion has good results, a less invasive technique, equally effective and safe, can be chosen, even in the proximity of neurological structures.

      Abstract: Systemic mastocytosis is a rare malignant disease involving mast cells and their CD34+ progenitor cells. Signs and symptoms are caused by the release of biological mediators from mast cells and by the infiltration of these cells into different organs. The skin and bone marrow are predominantly involved. There are only two cases reported in the literature of patients with systemic mastocytosis undergoing hip arthroplasty. The authors present a case from their experience, highlighting critical points, namely the choice of implant and the technical execution.

      Abstract: Study of the clinical results of posterior ankle arthroscopy in 21 patients with pain related to impingement. The following procedures were performed: os trigonum excision, decompression of prominent talar process, tenolysis of the flexor hallucis, removal of loose bodies and debridement of osteochondral lesions. An improvement in the AOFAS score was recorded. Despite the small sample size, the results of this technique are promising. The authors argue that it is a simple, reproducible and safe technique in the treatment of posterior ankle impingement.

      Abstract: Retrospective study that evaluated 150 patients followed at our institution for traumatic knee injury who presented radiography of the healthy contralateral knee in 30° flexion. The objective was to evaluate the 3 most commonly used methods of measuring patellar height (Insall-Salvati index, Caton-Deschamps index and Blackburne-Peel index) and compare them with a new measurement method introduced more recently that is based on different anatomical references, the patellar angle. It was concluded that there is great inter- and intra-observer variability in the measurement of these indices and that there is only a correlation between the Caton-Deschamps index and the patellar angle. Thus, these 2 indices appear to be those that best correlate with the height of the patella.

      Abstract: Study of 50 patients whose objective was to demonstrate that, with adequate selection of patients with prosthetic infection and a structured protocolized treatment regimen, it is possible to obtain excellent results. Patients with acute hip or knee prosthesis infection undergoing surgical treatment with debridement, without prosthesis extraction, were included. It was concluded that this procedure is an effective treatment as long as the assumptions in patient selection are met and some recommendations for surgical and medical treatment are followed.

      Abstract: Study of 39 patients that aimed to evaluate the main complications and the final result of surgical treatment of tibial pilon fractures. The study revealed that skin conditions determine the initial treatment strategy. The progression to osteoarthritis depends more on the severity of the fracture than on the surgical strategy. The clinical result is satisfactory, with most patients returning to daily life without significant limitations.

      Abstract: Instrumentation of fractures of the thoracic and lumbar spine with pedicle instrumentation systems allows their stabilization, correction of kyphotic deformity and restoration of vertebral height. The “load sharing” classification allows to identify fractures that require complementary anterior instrumentation. The objectives of this study were: to analyze the reduction capacity of these fractures with the isolated use of pedicle fixation systems; understand its behavior over time; identify patient or fracture factors that influence the reduction capacity, and its maintenance over time. It was possible to understand the effectiveness of these systems in the treatment of these fractures. An adequate reduction of the deformity of the fractured vertebra and its segment was observed, which is partially lost in the first 6 weeks. The gains achieved and the losses of correction are independent of the characteristics of the patient (except for age) or the fracture, suggesting that complementary instrumentation via the anterior route may be unnecessary in this type of fracture.

      Abstract: This study demonstrates that early stabilization of spinal fractures (less than 72 hours) in severe polytrauma patients is beneficial and shortens intubation time, intensive care unit stay, and overall hospital stay. No differences were observed in terms of mortality rate. Although some patients may require deferred treatment due to the necessary medical stabilization, all reasonable efforts should be made to perform surgery as early as possible.

      Abstract: Study of 41 patients with ankle osteoarthritis. The aim of this study was to compare the results of arthrodesis versus ankle arthroplasty. Patients undergoing arthroplasty reported better functional results and better pain control. It was concluded that both surgical methods used had satisfactory results. Arthrodesis alters the biomechanics of gait. Arthroplasty will be a more physiological alternative, with the disadvantage of higher costs and failure.

      Abstract: A retrospective study with 10 years of follow-up with a total of 150 patients with unilateral or bilateral olalgia in whom a bilateral ultrasound study was performed was presented. Shoulders with cuff rupture were recorded and divided into 2 groups: shoulders with symptomatic ruptures and shoulders with non-symptomatic ruptures. The same radiologist performed a new ultrasound study and it was found that there was greater progression of symptomatic and asymptomatic ruptures above 60 years of age. Overall, 64.29% of ruptures progressed after 10 years. Symptomatic ruptures show a greater increase in size compared to asymptomatic ones, and total ruptures are more likely to progress compared to partial ruptures. Surgical treatment of ruptures has proven to be effective with a low recurrence rate.

      Abstract: Seventy-one patients operated on for subaxial cervical fracture-dislocation over a 10-year period were retrospectively analyzed. The following parameters were evaluated: age, sex, injury characteristics (dislocation or subluxation, unilateral or bilateral, level of injury and presence of associated facet fracture), trauma mechanism, presence of spinal cord injury (American Spinal Injury Association Impairment Scale [ASIA]), surgical data and follow-up records. It was concluded that rigid spines and bilateral facet dislocations have a higher risk of spinal cord injury. After successful closed reduction, isolated anterior cervical fusion offers an excellent surgical option in the treatment of many cervical fracture-dislocations. Failure occurs most frequently at the C7-T1 level, suggesting that, here, a 360° fusion may be necessary.

When functionally evaluating these patients, it was concluded that there is scientific evidence to support the current trend of not treating these patients with osteosynthesis, favoring conservative treatment or reversed total shoulder arthroplasty.

      Abstract: This retrospective study was conducted between 2004 and 2014 and included all patients with an ICD-9 diagnosis of spondylodiscitis and those who underwent CT-guided needle aspiration biopsy for suspected spondylodiscitis with positive culture and/or pathological anatomy. The results of this study demonstrated that performing a biopsy before starting antibiotic therapy improves the possibility of identifying the agent involved. Needle biopsy should be part of the diagnostic tools for all patients with suspected spondylodiscitis, since the identification of the microorganism allows for more effective targeted treatment.

      Abstract: Retrospective study of 21 patients diagnosed with chondrosarcomas that aimed to evaluate treatment, survival, complications and functional results. Tumor resection remains the only effective curative treatment for all chondrosarcomas. However, in low-grade chondrosarcomas, intralesional or marginal resections may be adequate. This study presents 4 cases in which, to avoid the high morbidity of resection and improve functional results, it was possible to perform aggressive curettage, without causing tumor recurrence. The role of the orthopedic surgeon in the treatment of chondrosarcomas is crucial as he must perform surgery that is both curative and with the least possible functional impairment.

      Abstract: The clinical case of a 26-year-old patient with rigid equino-cavo-varus foot caused by sciatic nerve injury in childhood is presented. In this case, a superoexternal osteotomy of the tarsus, a valgization osteotomy of the calcaneus, lengthening of the Achilles tendon and arthrodesis of the MF of the hallux were performed, preserving the ankle. At 6 months, the osteotomies were consolidated, the mechanical axis of the ankle was aligned and the patient was able to wear normal shoes.

      Abstract: This paper reports on a difficult case that required several interventions and debridements that culminated in a 2-stage revision with a tumor megaprosthesis due to chronic infection. The impact that infection can have on limb preservation surgery, the importance of early diagnosis, adequate treatment strategy, accurate identification of the infecting pathogens and the appropriate antibiotic regimen are highlighted. Debridement and irrigation with preservation of the prosthesis are often the first attempt in these difficult cases, but their success depends on the short duration of symptoms, knowledge of the infecting microorganisms and the correct antibiotic therapy with anti-biofilm properties.

      Abstract: Study with 25 patients who underwent 26 consecutive metatarsophalangeal arthroplasties in the treatment of hallux rigidus. It was found that the medium- and long-term results of this new generation of prostheses seem to surpass the complications observed with the previous ones. The low rate of complications identified, associated with the preservation of foot mobility and biomechanics, seem to be related to patient satisfaction.

      Abstract: Study of 100 patients diagnosed with rhizarthrosis, which aimed to compare the clinical results between trapezectomy with ligamentoplasty, according to the Sigfuson-Lundborg technique, and trapeziometacarpal arthroplasty. It was found that there was no functional superiority of one technique over the other. It was concluded that the great advantage of arthroplasty with implant was the earlier return to daily activities. However, this technique is associated with higher costs and a higher risk of complications.

      Abstract: We present a rare case of progressive cervical kyphosis C4-C5 in a patient with Crouzon Syndrome, seen in the emergency department due to insidious onset of ASIA D neurological deficits in the context of a cervical kyphotic deformity. This deformity, documented and progressive for at least 5 years, was neglected by the attending physician. The patient was placed under cervical traction on a Stryker bed, and evolved with complete neurological recovery. Three days later, he underwent circumferential cervical arthrodesis: posterior compression with wire from C5-C6 and corporectomy of C5 and anterior arthrodesis C4-C6 with interposition of an autologous iliac crest graft and plate fixation. To our knowledge, this is the first case described of a progressive cervical kyphotic malformation in a patient with Crouzon Syndrome.

      Abstract: Study of 10 patients diagnosed with chronic post-traumatic anterior glenohumeral instability, with several episodes of dislocation, submitted to the arthroscopic Bristow-Latarjet procedure, by the same surgeon. In this work, a statistically significant improvement in the Constant and WOSI scores was observed, with no cases of recurrence. After 1 year, all patients returned to their professional lives without restrictions and the 6 athletes returned to competition.

      Abstract: The clinical and imaging results of a patient submitted to pedicle subtraction osteotomy of L3 and D9-iliac instrumentation due to loss of sagittal balance are presented.     • TREATMENT OF OSTEOBLASTOMA, EXPERIENCE OF A SERVICE

      Abstract: Seven patients followed in our service, aged between 12 and 42 years at the time of diagnosis, were reviewed, of which 3 represented axial lesions (one lumbar, one cervical and one iliac), and 4 limb lesions (one humerus, one fibula, one tibia and one talus), and in all of them the main complaint was continuous pain with no special nocturnal predominance. It was concluded that, despite being histologically a benign lesion, without cellular atypia, the inflammatory reaction caused by the lesion may suggest local invasion. The therapeutic option depends mainly on the extent and location of the lesion. In the cases reviewed, the main treatment was curettage, which has the advantage of being less traumatic and less invasive than wide excision. In more extensive osteoblastomas, and especially when they occur in the hand, foot, ribs, fibula or diaphysis of a long bone, wide excision may be preferable and more appropriate. Lastly, a prior biopsy allows us to rule out a malignant lesion and to adapt the treatment, which ideally will involve curettage with phenolization.

      Abstract: Two patients with chondrosarcomas were presented: 1 patient presented an osteolytic lesion of the middle third of the femur, with thickening and bulging of the cortex, and the other a cartilaginous lesion of the proximal humerus, with cortical invasion but without surpassing it. Curettage, phenolization and filling with cement were performed in both patients. This study aimed to demonstrate that this form of treatment has results, in terms of recurrence, that are similar to those of the classical approach, but presents better functional results and lower complication rates.

      Abstract: Presentation of a case of a 41-year-old woman who presented a palpable mass in the suprapubic region corresponding to a pelvic chondrosarcoma originating in the iliopubic branch in which an extensive resection of the lesion was performed requiring partial cystectomy. It was demonstrated that collaboration between different specialties is essential in the approach to oncological disease and only then is it possible to correctly plan and guide the treatment of these diseases.

      Abstract: Study of 19 children with osteoid osteoma. All underwent radiofrequency thermoablation guided by computed tomography, on an outpatient basis.       Abstract: Study of 30 patients with hallux rigidus stage III and IV (Coughlin and Shurnas Classification), which aimed to compare the results of arthrodesis versus arthroplasty of the 1st metatarsophalangeal joint. It was concluded that arthrodesis provides pain relief and satisfactory results, but alters the biomechanics of gait, whereas arthroplasty improves pain and is a more physiological alternative by maintaining the foot mechanics preserved. It has the disadvantage of higher costs and wear/failure over time.

      Abstract: Retrospective analysis of 55 patients undergoing treatment for liposarcomas of the extremities in a reference center and review of best practices in the approach to these lesions. The treatment is still not very specific, and the general principles of treatment for soft tissue sarcomas are adopted. Neoadjuvant radiotherapy is controversial. The emergence of new, more targeted and specific therapies may allow the treatment of those patients who can currently only be offered palliative therapies.

      Abstract: Study of 38 patients with chronic flexor tendon injury in zone II, undergoing tendon reconstruction in two stages according to the Hunter reconstruction technique. It was concluded that with the correct indication and technique, this procedure appears to be effective in restoring digital function in tendon injuries in the area of ​​the digital flexor sheath. It is a demanding procedure for the patient, who must be motivated to have the ability to complete a long rehabilitation program.

      Abstract: The clinical case of a 51-year-old patient with sagittal imbalance who underwent pedicle subtraction osteotomy at L3 and posterior pedicle instrumentation at D10-sacroiliac joints requiring 2 subsequent revision surgeries due to pseudarthrosis and stress fracture of the bars is presented. In the first surgery, the titanium bars were replaced by cobalt-chromium bars and, in the second, transforaminal interbody fusion was performed, superiorly and inferiorly to the level where the osteotomy was performed, and the number of bars was increased to 4. Pseudarthrosis is one of the possible complications after a pedicle subtraction osteotomy, generally leading to the need for revision surgery, which can be performed with clinical and imaging success.

      Abstract: This study aimed to evaluate and report our experience with the magnetic nailing and lengthening system in lower limb dysmetria. It was concluded that despite the higher cost of this surgery, the magnetic lengthening system achieved excellent functional results with unequivocal benefits in terms of pain, reduction in the number of subsequent surgeries, reduction in complications and greater comfort when dressing and performing daily tasks.

This clinical case of a 20-year-old patient with chronic ankle instability and talo-calcaneal tarsal bar demonstrates the need to treat both pathologies together in order to obtain good clinical results.

    • SURGERY AS A LAST RESORT IN SEVERE DEFORMITY OF CHARCOT DIABETIC ARTHROPATHY

      Abstract: A clinical case of a 62-year-old female patient with Charcot arthropathy with destruction of the tibiotarsal and Chopart joints, with subluxation and necrosis of the talus is presented. Prioritizing the recovery of skin conditions for future reconstruction of the foot and ankle, the patient underwent a long period of immobilization and unloading of the limb, with weekly evaluation and treatments by the multidisciplinary team. Eight months later, the patient underwent excision of the talus and tibiocalcaneal arthrodesis with a retrograde nail, with good clinical and radiological progress. This case shows that in the case of severe deformities, a differentiated multidisciplinary approach is necessary to enable the definition of the most appropriate strategy, which should only involve amputation as a last resort.

      Abstract: A prospective study was developed to evaluate the arthroscopic treatment (Bankart repair and/or capsulorrhaphy) of posterior shoulder instability in children. Of the 6 patients who underwent surgery, all showed improvements in functional results, with no recurrence of instability and a return to daily and sporting activities without limitations. Thus, the efficacy and safety of arthroscopic procedures in the treatment of this pathology were demonstrated, even in patients under 18 years of age.

It was concluded that the increase in the distance between the plate and the adjacent platform and the tip of the screws and the adjacent platform is related to the decreased risk of progression of ossification of the adjacent disc.

      Abstract: The authors retrospectively evaluated the clinical and radiographic records of all patients undergoing surgery for periprosthetic hip and knee fractures in a 4-year period, with the aim of identifying predictors of morbidity and mortality after surgical treatment of these fractures. It was concluded that recovery of walking ability, patient age and ASA classification were identified as factors associated with lower mortality per year.

    Resumo: Estudo retrospetivo desenvolvido entre 2004 e 2014 em que se incluíram todos os doentes com o diagnóstico ICD-9 de espondilodiscite e aqueles doentes que foram submetidos a biópsia aspirativa guiada por TC por suspeita de espondilodiscite com cultura e/ou anatomia patológica positiva. Os resultados deste trabalho demonstraram que a realização de biópsia antes de iniciar antibioterapia melhora a possibilidade de identificação do agente envolvido. A biópsia por agulha deve fazer parte das ferramentas diagnósticas de todos os doentes com suspeita de espondilodiscite, na medida em que a identificação do microrganismo permite um tratamento dirigido mais eficaz.
  • CONDROSSARCOMAS – A EXPERIÊNCIA DE UM SERVIÇO

    Resumo: Estudo retrospetivo de 21 doentes com o diagnóstico de condrossarcomas que pretendeu avaliar o tratamento, sobrevida, complicações e resultados funcionais. A resseção tumoral permanece como o único tratamento curativo eficaz no tratamento de todos os condrossarcomas. Contudo, nos condrossarcomas de baixo grau, as resseções intralesionais ou marginais podem ser adequadas. Este trabalho apresenta 4 casos em que, para evitar a grande morbilidade de uma resseção e melhorar os resultados funcionais, foi possível fazer uma curetagem agressiva, sem condicionar recidiva tumoral. O papel do cirurgião ortopédico no tratamento dos condrossarcomas é crucial pois tem de realizar uma cirurgia que seja ao mesmo tempo curativa e com o menor compromisso funcional possível.

    Resumo: Apresentado o caso clínico de um doente de 26 anos com pé equino-cavo-varo rígido causado por lesão do nervo ciático na infância, em que se realizou osteotomia superoexterna do tarso, uma osteotomia de valgização do calcâneo, alongamento do tendão de Aquilles e artrodese da MF do hallux, preservando o tornozelo. Aos 6 meses, as osteotomias estavam consolidadas, o eixo mecânico do tornozelo estava alinhado e o doente era capaz de calçar sapatos normais.

    Resumo: Este trabalho refere-se a um caso difícil, que necessitou de várias intervenções e desbridamentos que culminaram numa revisão a 2 tempos com uma megaprotese tumoral devido a infeção crónica. Realça-se o impacto que a infecção pode ter na cirurgia da preservação do membro, a importância do diagnóstico precoce, a estratégia de tratamento adequada, a identificação exata dos agentes patogénicos infectados e o regime antibiótico adequado. O desbridamento e a irrigação com a preservação da prótese são muitas vezes a primeira tentativa nestes casos difíceis, mas o seu sucesso depende da curta duração dos sintomas, do conhecimento dos microrganismos infectados e da terapêutica antibiótica correta com propriedades anti-biofilm.

    Resumo: Estudo com 25 doentes submetidos a 26 artroplastias metatarsofalângicas consecutivas no tratamento de hallux rigidus. Verificou-se que os resultados a médio e longo prazo desta nova geração de próteses parecem superar as complicações verificadas com as anteriores. A baixa taxa de complicações identificadas, associadas à preservação da mobilidade e biomecânica do pé, parecem estar relacionadas com a satisfação dos doentes.

    Resumo: Estudo de 100 doentes com o diagnóstico de rizartrose, que teve como objetivo a comparação dos resultados clínicos entre a trapezectomia com ligamentoplastia, segundo a técnica de Sigfuson-Lundborg, e a artroplastia trapézio-metacarpiana. Verificou-se que não houve superioridade funcional de uma técnica relativamente à outra. Concluiu-se que a grande vantagem da artroplastia com implante foi o retorno mais precoce às atividades de vida diária. Contudo, esta técnica está associada a maior custo e maior risco de complicações.

    Resumo: Apresentado um caso raro de cifose cervical progressiva C4-C5 num doente com Síndrome de Crouzon, observado em contexto de serviço de urgência por défices neurológicos ASIA D de início insidioso no contexto de uma deformidade cifótica cervical. Esta deformidade, documentada e em progressão há pelo menos 5 anos, foi negligenciada pelo médico assistente. O doente foi colocado sob tração cervical em cama de Stryker, tendo evoluído com recuperação neurológica completa. Três dias depois, foi submetido a artrodese cervical circunferencial: compressão posterior com aramagem de C5-C6 e corporectomia de C5 e artrodese anterior C4-C6 com interposição de enxerto autólogo de crista ilíaca e fixação com placa. Até onde sabemos, este é o primeiro caso descrito de uma malformação cifótica cervical progressiva num doente com Síndrome de Crouzon.

    Resumo: Estudo de 10 doentes com diagnóstico de instabilidade glenoumeral anterior pós-traumática crónica, com vários episódios de luxação, submetidos ao procedimento de Bristow-Latarjet artroscópico, pelo mesmo cirurgião. Neste trabalho, verificou-se uma melhoria estatisticamente significativa nos scores de Constant e WOSI, sem casos de recidiva. Ao final de 1 ano, todos os doentes voltaram à sua vida profissional sem restrições e os 6 atletas voltaram a competir.

É mostrado que o balanço sagital foi restaurado após a cirurgia, permitindo recuperar a lordose normal da coluna lombar e corrigir os mecanismos compensatórios.
